Mottling and Skin Changes Before Death: What It Looks Like

Mottling is a blotchy or marbled color change that can appear on the feet, knees, hands, and other areas when circulation slows near death. It's one possible sign. It doesn't appear in every person, and it can't predict the exact time of death by itself.

In the last days, the skin changes in a few recognizable ways. It grows cool to the touch, starting at the hands and feet. It can turn pale or take on a waxy, yellowish look. And it develops mottling: blotchy, netlike patches of purple, red, and blue, most often on the feet, knees, and hands.^[1][2]

These changes may move inward as circulation declines. The color change alone can't establish whether the person has pain. Assess the whole person and report signs of discomfort to the care team.

The medical name for the color change is cyanosis when it's bluish, and the pattern is sometimes called livedo. It’s one of the more reliable signs that a person has entered the active dying phase. It’s also one of the most upsetting to see if you don’t know what it is.

As the heart weakens near the end, it can no longer push blood strongly to the edges of the body. Circulation pulls inward to keep the vital organs supplied, so blood flow to the skin of the hands, feet, and knees drops. Blood pools and slows in those small surface vessels, and that pooling shows through the skin as the blotchy purple-red pattern.^[4]

That’s also why the skin may feel cool. In a person who’s expected to be dying, the color change may need no treatment beyond comfort care. A new, sudden, one-sided, painful, swollen, or unexplained color change needs clinical assessment because mottling has causes outside the dying process.

Mottling usually starts at the feet and the knees, then the hands, because those are farthest from the heart and the first to lose steady blood flow. Over hours it can spread up the legs and arms toward the body.^[2]

The soles of the feet often go first, turning dusky or purple. If you’re watching for it, the feet and knees are where to look.

Mottling can be subtle on any skin tone. On brown and Black skin, a change may appear as darkening, an ashen or gray tone, or a change in the palms, soles, lips, gums, tongue, or inner eyelids. Lighting and the person's usual skin tone affect what's visible.^[3]

The reliable places to look are the areas with the least pigment: the palms of the hands, the soles of the feet, the lips, the gums and the inside of the mouth, the tongue, and the whites and inner lids of the eyes. On darker skin these areas may look gray, whitish, or dusky instead of pink.

Pressing gently on a nail bed and watching the color return also helps. This is standard nursing guidance, and it matters: color changes that are easy to see on light skin are easy to miss on dark skin unless you know where to check.^[3]

Mottling itself is a visible change in circulation and isn't generally treated as a source of pain. It doesn't prove that the person is comfortable or unaware. Look at the person's expression, movement, breathing effort, and other signs, and ask the care team to assess any concern.^[1]

Use a light blanket if the person appears cold and avoid heating pads, electric blankets, or hard massage on fragile skin. Call the care team if the change is unexpected, the person isn't already receiving end-of-life care, or any sign of discomfort is present.

Alongside mottling, the skin usually turns cool, starting at the hands and feet and moving inward. It can become pale, or take on a waxy, slightly yellow cast. Some people sweat or feel clammy. Lips and nail beds may darken or turn dusky as oxygen levels fall.^[1][2]

In an expected death, these may reflect declining circulation and oxygen. The same changes can have other causes. Contact the care team if they're sudden, unexplained, accompanied by distress, or occurring in someone who isn't known to be near death.
